Professional Form Manager lets you put custom forms on your storefront and get each submission by email, with uploaded files included. If you already work from a fillable PDF — an application, agreement, or order form — upload it and the app builds a matching form, then fills a copy of your PDF with each submission. Forms without a PDF can generate one automatically. Review submissions in your admin, keep sensitive answers masked, and export to CSV. Spam checks are built in.
